こんにちは、ケニア事務所インターンの柴谷あかねです。ケニアでは例年に比べて随分遅れて始まった冬の時期で、昼は炎天下、朝晩は肌寒い日が続いています。
Hello, everyone. I’m Akane, I’m an intern from the NICCO Kenya office. It has been cold in the morning/night since it is currently the winter season here in Kenya.

さて、前回は、産婦人科棟の工事について取り上げましたが、今日は地域保健について取り上げたいと思います。
In the last article, I have shared about the construction of the maternal wing. In this article, I would like to introduce about the regional health system in Kenya.

ケニアでは、村々を巡回し、地域の人々の保健・衛生への意識向上の促進、健康状態の把握・記録を行うCommunity Health Volunteer（以下、CHV）と呼ばれる女性たちがいます。そんな活動を行う彼女たちを１日密着して追ってみました。
In Kenya, there are workers called Community Heath Volunteer (:CHV), who work on raising the awareness of hygiene and health and on the record of health conditions of the people at the communities. I would like to introduce one of those days of their work.

今日はPeninaさんとPriscillaさんの２人のCHVに同行しました。
Today, we are following the work of two CHVs, Penina and Priscilla.

１軒１軒の家をまわり、健康状態について質問をしたり、困っていることや分からないことなどがあれば相談に乗ります。
Visit houses one by one, and have a talk about their health conditions.

ケニアには、日本と同じように、母子手帳があります。妊娠初期の妊婦から５歳までの乳幼児の健康状態を記録した母子手帳は、検診記録、予防接種、子どもの成長、家族計画などの記録を把握するのに大切なツールです。
In Kenya, there are maternal and child health handbooks, as we have in Japan. All the record, from the early pregnancy to the health condition of the child after birth, is noted in this handbook, which is very important to assess the condition of mother and child.

日本が発祥と言われる母子手帳は、ここケニアでも、普及しています。

妊婦、新生児、幼児のいる家では、CHVたちは、母子手帳の記録から、きちんと定期検診に行っているか、子どもの成長が順調かといったことを確認しながら、母親へアドバイスを行なっています。
CHVs give an advice to the mothers referring from the record of the handbook.

CHVの地域巡回で行うことは、世帯訪問による病人の特定、病院の紹介、母子保健検診の対象となる幼児の特定、予防接種が必要な児童の確認、出産が近い妊婦へのサポート、家族計画の推進、手洗いの習慣・トイレ設置の推進、マラリア検査など多岐にわたります。
There are varieties of work at CHVs’ field-visit, such as identification of the sick, pregnant mothers and children in needs of immunization, support of pregnant mothers, implementation of family planning, implementation of the toilet/washing hand facility, testing of Malaria etc.

※この地域の多くの世帯には、水道が通っていません。CHVたちは、感染症を防ぐ為、新生児がいる世帯を中心に、キッチン、トイレへの手洗い設備の設置を促進しています。洗剤などの使い終わった空のポリ容器の下部に穴を開け、穴を木の枝で塞ぎます。容器を縄で吊るし、水を入れ、木の枝の抜き差しすることで蛇口の役割を果たす、手洗い設備の完成です。子どもたちも簡単に使うことができます。

この日は、３時間で10件ほどの世帯を訪問しましたが、炎天下の中、家々を歩いて回るのは大変でした。
On this day, we have walked for 3 hours, visiting around 10 households. It was tough to walk around under the burning sun.

以上、CHVの活動の様子をご紹介しました。
しかし、彼女たちの中には、現在、行政の資金的な問題によって、この活動に必要な知識を得る為のトレーニングが受けられていないボランティアもいます。また、10年以上も前にトレーニングを受けたきり、その後１度もトレーニングを受けていないボランティアもいます。自分たちが持つ知識を地域に精一杯還元し、役立てようというCHVたちに対し、この活動へのサポートが追いついていないのが現状です。
This is the work of CHVs. However, there are CHVs who have not taken the training to get the knowledge needed as CHV, because of the lack of the government budget. There are also CHVs who have not taken the training since 10 years back and have no chance to update the knowledge. CHVs are willing to work for the community for the improvement of the health situation; however, the reality is that there is not much support for CHVs work.

そこでNICCOは、今年の秋から、母子保健に焦点を当てたCHVたちへのトレーニングを実施する予定です。
「自分が暮している地域の人々の健康に貢献できる仕事だから、この仕事に関わっていることは、私にとって嬉しいことなの。」というPeninaさん。彼女を始めとし、CHVたちにとって、そして地域の人々にとって、NICCOが貢献できることを願っています。
Therefore, NICCO is organizing training session for CHVs, focused on maternal and child health. Hope the work of NICCO contributes to the work of CHVs and for the community people.
